Privacy:

Dr. Liang honors your privacy highly. Her office and her staff respect the privacy of you and your family. Without your written consent, Dr. Liang will not release your medical information to any unauthorized party.

HIPPA Omnibus Rule which has been revised and has been in effective from 9/23/2013 is strictly complied. Every patient of Dr. Liang shall be given details on patient’s rights with regard to the protected health information ("PHI"). Every patient can request a copy of the Notice of Privacy Policy after signing.

However, for emergency and special situations, such as an imminent danger to yourself and others, you lost your ability to take care of yourself, suspected child and elderly abuse and other reportable situations, Dr. Liang is obligated to override your privacy right.

If you are unable to keep a scheduled appointment, please give at least 24 hours notice in order to avoid paying $25.00 no-show fee.

The returned check:

The charge for a returned check is $25.00.

Others:

There is a fee of $45.00 for completion of FMLA and/or short-term disability paperwork and/or any letters written on your behalf for your lawyer, employer, or pre-operation verification of health status.
